The focus of stroke rehabilitation at St. Charles is the patient and providing him/her with the therapy needed to reach their maximum potential. Upon admission, patients who have suffered a stroke are evaluated by the rehabilitation team. Then an individualized plan of care, that addresses each patient's specific issues, is developed. The process begins with acute rehabilitation and includes occupational and speech therapy. As patients begin to progress, the program expands to incorporate community re-entry to facilitate patients' achieving the necessary goals to meet the discharge plan.
A comprehensive and collaborative team approach is utilized with a board-certified physician specializing in physical medicine and rehabilitation (physiatrist). The stroke rehabilitation team includes rehabilitation nurses, physical therapists, occupational therapists, speech and language pathologists, neuro-psychologists, respiratory therapists and social workers. The team meets weekly to discuss patients' progress towards reaching their discharge plan. Family participation is welcomed and encouraged throughout the rehabilitation process.
