Rehabilitation Admissions
Patients admitted for inpatient rehabilitation usually come to St. Charles directly from an acute care hospital. Following a physician's referral for rehabilitation, an admissions coordinator at St. Charles will facilitate the admissions process with the patient's discharge planner/social worker.
Patients who need inpatient rehabilitation should be referred as soon as they are medically stable and able to participate in an active rehabilitation program. This includes three hours per day of physical/occupational therapy.

Admission
After an admission decision and insurance verification are complete, arrangements are made for the patient's transfer. Patients should bring their own comfortable clothing and basic toiletry articles.
Discharge
As soon as a patient is admitted for rehabilitation, social workers and case managers work directly with the patient and family in planning for discharge. A discharge plan is drawn up for each patient based on his or her individual needs. Referrals may be made for home care, equipment and other necessary services
